What is Peterborough Wellbeing Service?

A vibrant local scheme working closely with the local health and social care system, including statutory services and the private sector.

  • Working with older people and their carers.
  • Working with people with long term health conditions.
  • Working with Countywide groups to address social isolation and loneliness.
  • Maximising health and wellbeing opportunities.

  • Helping to support Admission Avoidance, Delayed Transfer of Care and supporting patients returning home from hospital

  • Coordinating Multi-disciplinary team meetings for the Voluntary Sector across Peterborough

Future Plans:

Looking  to provide 10 pilot schemes for group exercise and nutrition for residential homes, BAME, faith and disability groups.
